Efficacy of Large Loop Excision of the Transformation Zone(LLETZ) in the Diagnosis and Management of Cervical Pathology




Abstract
Long loop excision of the transformation zone(LLETZ) of cervix is being widely used for the diagnosis and management of cervical lesions because of its many advantages over conventional knife conization.
This study was undertaken to determine the effect of large loop excision of the transformation zone on cervial pathology by evaluation of various parameters, such as operating time, safety, blood loss, effectiveness, complication and
histopathologic
accuracy.
Seventy two women who visited outpatient clinic with cervical pathology from May to November 1993 were treated with LLETX under local anesthesia on outpatient basis.
Age distribution was from thirty to sixty broadly with mean 44.7¡¾12.42. Mean operating time was shorter than by previous other methods such as cold knife conization, laser conization, and also mean blood loss was measured lesser in amount.
Vaginal
spotting or bleeding was similar in occurance compared with other methods, no serious complications were seen.
The surgical margin involvement of LLETZ was seen in 33.3% but the residual lesion after hysterectomy was not seen in 532.8%. The rat se of surgical margin involvement was somewhat higher than other studies, necessiating methodological
modification.
In conclusion, LEETZ is a more effective, quicker, safer and more reliable procedure on diagnosis and management4 of cervical pathology.
